Reablement support across Hampshire
Reablement is short-term, goal-focused support designed to help people regain their independence after illness, injury, or a hospital stay. Our approach is built around what you want to achieve - not just what care we can provide.
What is reablement?
Reablement is different from standard homecare. Rather than doing thingsfor you, our carers work alongside you, building your skills and confidence to do things yourself. The goal is maximum independence in minimum time.
Support is typically provided for up to six weeks and is reviewed throughout. Where ongoing care is needed after the reablement period, we can transition seamlessly to long-term domiciliary or live-in care.
Reablement is particularly effective following a fall, fracture, stroke, surgery, or prolonged hospital admission.
Common reablement goals
- Regain ability to manage personal care independently
- Build confidence following a fall or injury
- Develop safe routines for medication management
- Re-establish mobility and strength after surgery
- Resume domestic tasks and cooking
- Prepare for any longer-term care if required
